Summary:
Direct Support Professionals supervise adults with disabilities and ensure the needs of the individuals are met including physical, social, and emotional.
The DSP evaluates individual performance; assists with physical mobility, feeding, service plan supports, toileting and bussing needs.
Essential Job Duties and
Responsibilities:
Vocational Habilitation Trainer o Provide training to adults with developmental disabilities in the production of goods and/or services.
o Implement Individual Service Plans.
o Assign jobs based upon individual plan, monitor progress toward skill development and provides recommendations to individual service plan as needed.
o Work with Employment Manager/Assistant to ensure quality production goals, contract goals and objectives are met.
o Resolves and coordinates solutions of problems between consumers, supervisors, and other personnel.
Provides constructive feedback to teach individuals as required.
Community Inclusion Staff o Provide and document opportunities daily for community integration.
o Plan and assist individuals to engage in activities or outings.
Support and enhance community linkage.
o Implement Individual Service Plans.
o Transport individuals to and from activities.
Non-Medical Transportation Provider o Meets DOT physical requirements.
o Must be willing to complete proper safety training and learn to tie down wheelchairs.
o Operates passenger van or other motor vehicles to transport individuals with disabilities to and from Programming Site; assist individuals on and off vehicles as required; serves as liaison between homes and Sandco (i.
e.
, delivers notes, medicine, etc.
); administers first aid as required.
Ensures seat belts and other approved restraints are fastened, wheelchair lock downs are secure, and required wheelchair straps are in place.
Provides and documents services and supports according to Individual Service Plans.
o Performs daily safety and maintenance checks on van or other motor vehicles (e.
g.
, checks tire wear and tire pressure, checks oil, fuel, brake fluid and coolant levels, power steering, heater, defroster, wipers, etc.
); visually inspects van to ensure proper operation of safety equipment (e.
g.
, turn signals, taillights, brake lights, stop signal, flashers, headlights, parking lights, etc.
); reports need for vehicle maintenance and repair immediately.
Ensures vehicle has enough fuel for travel and is clean.
o Ensures the health and safety of all passengers.
Maintains van roster with current emergency information; takes attendance; maintains current map of route.
Completes accident and unusual incident reports.
o Prepares and maintains records and reports as required (e.
g.
, mileage log, vehicle maintenance reports, fuel consumption records, etc.
); maintains van cleanliness; maintains inventory of emergency equipment and supplies.
Documentation o Complete accurate daily documentation; consumer production and time sheet records, safety, unusual incident documentation and other record keeping and reporting procedures.
Complete additional tasks and responsibilities as needed or assigned.
Required Skills/Abilities:
Must have reliable transportation, a valid driver's license, acceptable driving record, and automobile insurance.
Bureau of Criminal Investigation Check must result in no felony record.
Obtain and maintain the Adult Service Worker training as required by DODD.
Education and
Experience:
High School Diploma or GED Operation of computer, phone and office equipment Must be able to handle confidential information with complete discretion.
Must maintain valid Driver's License.
Current CPR/First Aid Physical Requirements:
Must be able to operate hand cart, scales, and small hand tools.
Lift to 50 lbs.
Physical exam must result in being able to perform the essential functions of the position.
Drug screening must result in no substance abuse.
